Dehydration happens when your body loses more fluids than you can replenish. You might lose a lot of body fluid through sweating, breathing, urination, or illnesses that cause vomiting or diarrhea. When dehydrated, your blood volume can fall, and your body may struggle to deliver oxygen and nutrients to organs. Electrolytes (like sodium, potassium, and chloride) also shift, which affects how muscles and nerves function.
The Link between Dehydration and Stomach Pain
Here are different ways dehydration can cause stomach pain.
Indigestion and Constipation
Fluids, such as water, help your body digest food and push it through the entire gastrointestinal system. When you don’t take enough fluids, digestion slows down, stools may become hardened, and constipation can set in. That pressure in the colon often leads to cramping or pain in the lower abdomen.
Gut Muscle Cramps
Your gastrointestinal system is lined with muscles that contract in waves (peristalsis) to move the digested food (or being digested) along your gut. Dehydration and imbalance of electrolytes, such as potassium and sodium, can cause these muscles to spasm or contract irregularly. This can lead to cramp-like sensations or pain throughout your gut, including your stomach. Sometimes, electrolyte imbalances can cause dizziness and nausea.
Reduced Blood Flow and Ischemia
Severe dehydration can lower your body’s blood volume (hypovolemia). This reduces blood flow to organs, including the digestive tract. In extreme cases, such reduced perfusion may cause abdominal pain or even ischemic injury in the gut. Some surgical and emergency studies have documented “dehydration-related abdominal pain” in patients who improved after rehydration.
In addition, instances of dehydration reducing blood flow into the digestive tract have been seen in older people and endurance athletes, like marathon runners. Crampy abdominal pain is a common symptom of reduced blood flow into the gastrointestinal system.
Nausea, Vomiting, and Discomfort
When you are dehydrated, you’ll feel thirsty and weak. You may also experience stomach or abdominal cramps. If you don’t get enough fluids on time, you will begin to feel nauseous, vomit, and find it difficult to naturally consume fluids. Remember, some causes of dehydration, like bacterial or viral illnesses (with diarrhea), can also cause abdominal pain, vomiting, and excessive nausea.

- Persistent abdominal cramps or discomfort
- Dry mouth, thirst, or decreased urination
- Dark yellow urine
- Fatigue, dizziness, or lightheadedness
- Muscle cramps throughout your body
- Nausea or vomiting
If stomach pain is severe or does not improve after rehydration, medical evaluation may be necessary. You should also seek immediate medical attention if you cannot keep fluids (due to vomiting) or you’re experiencing sharp pain that seems to be worsening.
